Transitioning to
Community Living

Our Transition to Community Living services ensure ongoing contact and support between the rehabilitation services provider and the client, making the move from a hospital, residential treatment program (per Minnesota Rules, chapter 9505), board and lodging facility, or nursing home as smooth as possible. These services are focused solely on helping clients successfully transition back into community life and are not meant to replace other areas of adult rehabilitative mental health services.

To create positive change and support greater independence, we may work directly with agencies, institutions, employers, landlords, or family members — with or without the client present. Community Intervention is always tailored to the individual and can take place either in person or over the phone.
