Intervention

Our Community Intervention services are designed to remove barriers that stand in the way of a person’s ability to live independently and be an active part of their community. These services also help reduce the risk of hospitalization or the need for more restrictive living arrangements.

To create positive change and support greater independence, we may work directly with agencies, institutions, employers, landlords, or family members — with or without the client present. Community Intervention is always tailored to the individual and can take place either in person or over the phone.
